Description: Different physicians on different days. We shared 6 rooms with a NP and this facilitated seeing patients simultaneously without feeling too rushed as a student. All the rooms had the same equipment available. Clinic staff, MAs, were great at making sure everything was set up for particular exams – clinic went very smoothly.
Patient Profile: There is a full range of family practice patients except Obstetrics/pre-natal. This is a busy private practice with 3 physicians and 3 nurse practitioners.
Languages: Most patients are English speaking with a few Spanish speaking patients and some Japanese speaking patients.
Logistics: Tranquil community off 80, close to freeway. Clinic is on a busy street with lots of places to eat and Asian mall nearby.
Parking : Clinic has parking lot in front – always spots available.
Near Central exit off Interstate 80 as well as across the street from El Cerrito BART.
Special Directions : Take 80 North past Berkeley. Exit Central Ave, turn right. Continue through 3 or 4 lights until San Pablo Ave, turn right and you will see Albany F.P. (Alta Bates) on right hand side a few blocks down in front of mall.

Description: Asian Health Services is a primary care community health center located in Oakland Chinatown serving the indigent Asian and Pacific Islander population of Alameda County.
Patient Profile: Asian Health Services provides medical care and other services to a diverse immigrant patient population. Ninety-seven percent of the patients are below 200% of the poverty line and 68% of the patients live below the poverty line. Forty percent of the patients are on Medi-Cal, 16% have Medicare coverage and 41% are uninsured.
Clinic Staff: The clinic has a staff of over 120 including 16 providers (physicians and physician assistants) and seven nurses. Asian Health Services is also a community center for public health projects.
Language: The clinic provides care in nine languages, with the predominant languages being Chinese and Vietnamese. Students should speak at least one additional language (Mandarin, Cantonese, Vietnamese or Korean).

Student Comments: A noteworthy feature of the clinical experience at this clinic is that the preceptor who the medical students works with may or may not be scheduled for patients of his or her own, in which case the preceptor would have more time to spend with the student.

Description: This is the largest private opiate dependency treatment clinic in California, with up to 800 patients. It is entirely outpatient, with hours from 7 am - 3 pm M - F.
Psychosocial: This is a very big part of the BAART-Geary Clinic experience. There is in-depth exposure to opiate addiction in patients’ lives. Clinic uses team approach to treatment including counselors.
Patient Profile: The patients seen are adult and geriatric (anyone over 18). Our patients are opiate dependent,(heroin and other opiates) and are here for treatment of their opiate dependence and to receive primary
care services. Pregnant opiate addicts receive additional
services. This is an inner-city clinic located in San Francisco's Tenderloin district. Many are homeless or maginally housed; many have dual diagnoses (psych) and many have a wide variety of other medical problems, including those problems found in any patient population as well as those problems more often seen in injection drug users. Many patients abuse or are dependent upon other drugs and alcohol.
Clinic Staff: The medical department is staffed by one physician (internal medicine residency and substance abuse fellowship), two to three mid level providers
(NP/PA) and a medical assistant. Other staffing is provided by counselors, dispensing nurses and administrative and clerical personnel.
Languages: Only English is required. Other helpful languages are Spanish, Russian, Farsi and Chinese.
Logistics: Parking is available near intersection of Van Ness and Geary next to a restaurant called “Tommy’s Joynt”. Lot requires exact change. There is some street parking on Geary.

Description: Haight Ashbury Free Medical Clinic offers primary health care, as well as specialty care including comprehensive HIV treatment services, chiropractic, HIV and Hepatitis C testing, acupuncture, massage, and community information and referral services.
The Clinic serves a vital community function, acting as the family physician to its clients. The Free Medical Clinic welcomes everyone who walks through the door. All medical services provided by the clinic, including laboratory testing and medications, are free of charge and patients are encouraged to donate whatever they can towards their care. The facility includes an on-site pharmacy consisting of donated medications and a laboratory facility capable of simple testing. However, the Free Medical Clinic is unable to treat emergency medical problems, such as broken bones, severe bleeding, trauma, abdominal pains, etc. These and other emergency medical problems should be seen immediately at a hospital emergency room.
Patient Profile: The patient population in this clinic tends to be young although on a given day patients could range in age from 19 to 53, with an average age of 31. Very few of the patients have private medical insurance. Although the majority of the patients are employed in jobs that do not provide medical benefits some are homeless and/or unemployed. The majority of the patients come in for acute care, predominantly upper respiratory infections and dermatological complaints, but a significant number of them presented for care of more chronic complaints including hypertension, diabetes, asthma and issues related to substance abuse. Patients are seen on both an acute care and continuity basis.
Clinic Staff: There are several volunteer physicians of diverse training backgrounds, nurse practitioners, physician assistants, and medical students rotating through from various medical schools throughout the country.
Logistics: This legendary clinic is located on the corner of Haight and Clayton Streets in the Haight-Ashbury district.

Description: The Native American Health Center is one of two Native American Health Centers in the Bay Area (the second is in the Mission District of SF). Students are assigned to the Oakland center. Services provided by the centers include medical and dental services, WIC, mental health counseling, drug and alcohol prevention, and youth empowerment.
Patient Profile: There are over 40,000 patient visits per year to the two clinics. Although the Native American Health Center focuses primarily on improving the health of Native Americans, a diverse ethnic population receives care at the clinics. The Native American Health Centers provide outpatient health care to low income clients.
Clinic Staff: The Oakland clinic is staffed by 2 physicians and 4 mid-level providers.
Language: No language requirement.

Description: This is an impressive practice, which runs a non-profit organization and also research projects.
Patient Profile: Oasis specializes in the care of patients with Hepatitis C. Population is predominantly African American. Many patients have a history of IV drug use. Primarily IV drug use, MediCal, MediCare, uninsured with Hepatitis C, many co-morbid conditions, alcohol.
Clinic Staff: Solo practice – 1 nurse, 1 administrative assistant. As part of OASIS group, comm. board of directors, representatives, volunteer public health physician & social worker/study coordinator. Waiting area, 2 potential pt. rooms, lab/kitchen, and one office combined with exam room.
Languages: English predominantly
Logistics: Telegraph/West Oakland. The practice is located along a commercial boulevard between Oakland & Berkeley near Summit Hospital. Bus lines are available from MacArthur BART but a car for safety concerns is recommended – availability of parking behind clinic.
Schedule: Monday to Friday 9 am to 5 pm, biweekly Hepatitis C noon support groups Monday and Wednesday.
Student Comments: Many opportunities to practice procedures including skin biopsies, absess drainage and peritoneal taps with encouragement and support of preceptor.

Students are assigned to work with board-certified family physicians in private family medicine practices in San Francisco, East Bay and Peninsula communities. Practices generally include all age groups, from newborns to geriatrics. A wide spectrum of care for all minor and major medical problems is offered including routine visits for health care maintenance, office surgery, gynecologic care, and obstetrics. Students have ample opportunity to see patients under the supervision of the family practitioner. Depending on the level of the student's expertise and comfort, s/he will initially see and evaluate patients with acute problems, present the case, formulate a diagnosis, and arrive at a plan for treatment. Working in a private physician's office provides an alternative setting for students to gain clinical experience. In a private physician's office, students will come in contact with a varied patient population, see a wide range of health problems, and be exposed to the delivery of health care in a private setting.

Description: The Richmond Health Center is a very busy primary care public health clinic. This site features excellent teaching in a community clinic setting and is often combined with another East Bay clinic allowing students to spend a majority of their FCM 110 in the East Bay.
Patient Profiles: The clinic serves the underserved in Richmond and provides health care to mostly indigent patients of Contra Costa County. The clinic population's insurance coverage is about 10% Medicare, 50% Medi-Cal, and 40t General Assistance. Ethnic diversity is a hallmark of this clinic with an ethnic breakdown of about 50% African Americans, 15% Southeast Asians (Vietnamese, Laotian, and Cambodian), 15% Hispanics, and 15% other.
Clinic Staff: It is a family practice site staffed by family practitioners, internists, pediatricians, and nurse practitioners, with good support from the in-house specialists, pharmacy, radiology and laboratory facilities. The physicians are scheduled for 16 patients each four-hour session and the medical student may see 3-5 patients depending on the day.
Logistics: Just off Route 80 in Richmond in the East Bay. It is about a 30-minute drive from SFGH, with ample parking.

Description: The Clinic is the northern-most site of the San Mateo Medical Center clinic system. It opened in 1995 as a polyclinic offering full pediatric, adult, and psychiatric services. We also have specialty HIV and TB clinics and plan to offer a full range of Prenatal services by 2005. We have been teaching and precepting since we opened.
We are happy to teach students from any year. Special rotations in Tuberculosis and HIV can be arranged. Experience has taught us to involve the student as fully as possible with emphasis on exact fundamentals, the oral presentation of a case--an absolute essential as a foundation for strong, organized medical thinking, following a case into the home, the community, the hospital, the pharmacy, the supermarket. Notebook-keeping is expected, use of evidence-based resources encouraged.
Patient Profile: Children and Adults from diverse ethnic backgrounds including Southeast Asians-especially Filipinos, Latin Americans and Central Americans, South Americans
Clinic Staff: The providers include Family Practice, Internal Medicine, Family Practice Nurse Practitioners, Dentist.

Description: Founded in 1956, St. Anthony Free Medical Clinic provides primary and urgent care, as well as onsite specialty services including podiatry, psychiatry, allergy/asthma services, HIV medical care and testing, smoking cessation services, and health/nutrition counseling. Currently, we are the only free medical clinic in the Tenderloin that provides pediatric care. The Clinic also has an onsite CLIA-licensed laboratory and dispensary, which not only saves valuable time and money for our patients, but also ensures that our patients actually receive the tests and medications they need. All Clinic visits, including laboratory, radiology, and pharmacy services are provided free of charge. We serve between 350-400 adult patients every month through both scheduled and drop-in visits.
Patient Profile: We see the uninsured of SF. Homeless and indigent. Ages newborn to 55-60 years old. Primary and urgent care. No pre-natal or OB. This is a multicultural facility with Latino, Cantonese speaking Chinese, Vietnamese, Thai, Lao, Mien, Mongolian, etc.
Clinic Staff: We employ 21 paid staff members and several volunteers. Our paid staff includes six part-time physicians-including a podiatrist and pediatrician-one full-time physician, a family nurse practitioner, and 4 full-time registered nurses. Our volunteer medical staff includes a pediatric nurse practitioner and doctors in Orthopedics, Internal Medicine, Allergy/Asthma, and Psychiatry. Clinic staff are able to provide medical care in Cantonese, Lao, Mandarin, Mien, Portuguese, Spanish, Thai, Russian, and Vietnamese.
Languages: Students must be able to speak at least Spanish. Knowledge of Cantonese or Vietnamese also helfpul.
Logistics: MUNI/BART - This is a Civic Center stop. There are 2 parking lots available for those who drive at a cost of approximately $6 per day.

Its mission is to provide quality health care and social services to alleviate the suffering or homeless people, to promote their self-sufficiency, to advocate for their reintegration into society, and to work toward the ultimate elimination of homelessness.
Patient Profile: 41% homeless; 69% male; 91% ages 25-64; 42% Caucasian; 35% African American; 15% Latino; 90% English-speaking; 7% Spanish-speaking (FY01).
Clinic Staff: The clinic has over 100 staff, including 11 physicians and 12 nurse practitioners who provide primary and urgent care at the health center and medical services at numerous community sites. Description: UCSF Senior Medical Center at Lakeside is a satellite practice of UCSF Medical Center. Lakeside provides primary care for adults 65 years and older. Students make home visits to see patients living at home or in residential care homes for the elderly. Patient care is coordinated with home health, care management, and hospice agencies. Group practice with several rooms – most students go on home visits.
Patient Profile: Geriatric patients, some active, others homebound; private insurance and Medicare.
Clinic Staff: Lakeside is staffed by a family physician and a geriatrician internist.
Languages: Primarily English.

Student Comments: A great learning experiences about Hospice, home health nurses, board and care facilities, end of life issues, and continuity of care in a related atmosphere. Seeing people in their homes is a valuable opportunity not much available to medical students.
